Both Catherine-B320 and Carter-A259 took responsibility for his death, unable to admit that his death was his own fault. During the mission, he had chosen to go solo and attack Covenant forces rather than wait for backup.[1] Unfortunately, his replacement in Noble Team, SPARTAN-B312, has a reputation for the same "lone wolf" tactics that got Thom killed.
