I did this on co-op, and my secondary account died, but my primary one survived the whole level and I didn't receive the achievement. Any help? -Guardax

In the tips level of this article, it says:

When playing in cooperative mode, if one of the players die, the player that manages to survive the entire level will be the only one to unlock achievement.
