Theory

Given the fact that the platform normally docked foor levels above the Chamber is seen destroyed, could it be possible that 2401 had already located a Reclaimer and was in the process of retreiving the Index when the Flood interfered? Perhaps this is how the Gravemind captured 2401, and in the process, Flood Combat Forms destroyed components of the interior of the Library.
