Picture taken in Halo 2 Anniversary level "The Oracle", but can also be found on level "The Arbiter". I personally found this Unggoy from repeatedly playing these two levels on Legendary until he spawned from the first Phantom on "The Oracle" following the Flood introduction of the level (after going down the elevator and surviving the rooms following). Upon switching to classic graphics mode he wears the Pointed Ultra Combat Harness, but since only Special Operations Covenant were deployed on these two missions (along with the Arbiter, Thel 'Vadumee) this is proof that Special Operation Unggoy Ultra Rank exists.
